Issue and exchange of disability certificate

ISSUE AND EXCHANGE OF DISABILITY CERTIFICATE 

Individuals for whom a working capacity, a degree of disability or a degree of special needs is established are issued with a disability certificate by the territorial divisions of the Disability and Working Capacity Assessment Office under the Ministry of Social Security and Labour (hereinafter referred to as the DWCAO) that are responsible for performing an assessment of a degree of disability, a working capacity and of special needs. These divisions are also responsible for exchanging  disability certificates.

Issue of certificates

Applicants are issued with a disability certificate (or a certificate is sent to them) together with a decision of the territorial division of the DWCAO regarding the establishment of a degree of disability, a working capacity or a degree of special needs.

A certificate can be collected by the applicant himself or herself, and when he / she is unable to do so, the certificate can be collected by the applicant‘s parents, spouse, adult children, custodian (guardian) or other authorized persons, or by a social worker of the municipality of residence who represent an applicant, or by an employee of a social institution in which an applicant lives. An individual who provides a document certifying authorization (a number of an authorization must be provided when an electronic authorization is submitted via the Register of Authorizations) is considered to be an authorized person.

Exchange of certificates

A disability certificate shall be exchanged if:

  • holder of the disability certificate changes his / her forename (forenames), surname;
  • an old certificate has become unusable;
  • a certificate contains inaccurate entries;
  • in case a certificate was lost;
  • in case a new certificate is issued.

The following documents should be provided by an individual who wishes to exchange a disability certificate:

  • an application for exchange of a disability certificate that must specify a motivated reason for exchange. If a recipient of a disability certificate does not provide an old certificate, in such case an application of a recipient of a disability certificate must specify a reason (reasons) for non-provision of a certificate;
  • Personal Identity Card or a residence permit in the Republic of Lithuania, and/or a birth certificate if a child was born before 1 January 2017, a document certifying the identity of an authorized person (a passport or Personal Identity Card of the Republic of Lithuania, or a passport, or a residence permit in the Republic of Lithuania, except when an electronic service of this type is provided via E-Government Gateway system and the applicant applies online for exchange of disability certificate.

When an individual lodges an application electronically, such the applicant must confirm his / her identity by means of electronic signature or by other secure means (for example, via online banking system);

  • a photo with a size 3 x 4cm;
  • an old disability certificate.

If a holder of a disability certificate applies for the purpose of exchanging his / her disability certificate because he / she has changed his / her forename (forenames), surname, an old certificate has become unusable or a certificate contains inaccurate entries, an old certificate must be returned by its holder within 10 working days of receipt of a new certificate.

The certificates issued in Municipal Social Support Departments until 31 December 2018 shall be exchanged taking into account the information received from Municipal Social Support Departments on a degree of special needs established for that particular individual.

What should a holder of a disability certificate do if he / she has lost his / her certificate? 

An individual who has lost or has been for other reasons deprived of his / her certificate of disability must apply to territorial division of the DWCAO for a replacement certificate, and must submit:

  • a written motivated application for a replacement document;
  • a personal identity document (a copy of a personal identity document duly certified in accordance with the legal acts must be enclosed to an application that is sent by post);
  • a photo in black and white or in colour with a size of 3x4 cm.

The disability certificates issued until 31 December 2018 are valid until they are exchanged by their holders (except for the disability certificates issued to individuals under 18 years of age that are valid until the maturity deadline of disability).
